~ Daniel Lessler, MD, MHA, the Associate Medical Director at Harborview Medical Center in Seattle, Washington, and Pamela Piering, the Director of Aging and Disability Services for Seattle-King County, led a workshop for the 2007 Fellows on developing successful strategic partnerships.

~ The National Advisory Board discussed the former HMO Care Management Workgroup reports.

~ Advisory Board Member Dave Gans led a web-based seminar for Fellows on Benchmarking:

"Benchmarking: Using Internal and External Data to Measure Performance" presentation.

~ Several Fellows presented case studies of their projects:

~ Advisory Board Member Cheryl Schraeder spoke to the Board about the Medicare Chronic Care Practice Research Network

~ Marshall Chin, MD, MPH, director of The Robert Wood Johnson Foundation's "Finding Answers: Disparities Research for Change National Program Office" and the co-director of the Hartford Center of Excellence in Geriatrics at the University of Chicago, discussed approaches to organizing and delivering care to reduce disparities.

~ Advisory Board Member Cheryl Phillips led a web-based seminar for Fellows on Running Effective and Productive Meetings:

"Developing Effective Leadership Tools: Making Meetings Work & Creating an Effective Agenda and Managing your Meeting for Success" presentation.

~ The topic of the advisory board seminar was Palliative Care. Below are resources from that seminar:

The "Home Based Palliative Care" presentation by Kaiser Permanente's Richard Brumley, MD, and Gretchen Williams, MSW.
A sample script, provided by the Kaiser team, to help doctors introduce palliative care to patients and families.

~ Fellows were given guidelines for their semi-annual financial and progress report, due March 31, 2008. A financial template was also provided.

~ The agenda from this meeting is an example of the typical structure of the tri-annual meetings.

~ Robert Berenson, MD, Senior Fellow at the Urban Institute, was our keynote speaker. His presentation, "The Emerging Challenge of Chronic Care", was based on a paper he wrote for the California HealthCare Foundation: "Challenging the Status Quo in Chronic Disease Care: Case Studies".

~ Advisory Board member Judy Black presented a case study on Highmark's Transitions of Care, A Medicare Advantage QualityBLUE Pay for Performance Model.

~ Dennis A. Ehrich, MD, FACC, Vice President for Medical Affairs at St. Joseph's Hospital in Syracuse led an Outcomes Measurement Seminar for the Fellows. Below are materials from his seminar:

~ Many members of the Advisory Board were formerly part of The HMO Workgroup on Care Management. Over seven years, this group published eight reports on ways in which the delivery of care to Medicare beneficiaries can be improved.

~ Various reports were discussed as valuable resources during the meeting. These included:

The June 2007 MedPAC Report (specifically Chapter 5)
Healthy IDEAS: A Depression Intervention Delivered by Community-Based Case Managers Serving Older Adults, an article co-authored by Advisory Board member Nancy Wilson that was published in the April 2007 issue of the Journal of Applied Gerontology
Advanced Illness Care Coordination in a Medicare Advantage Setting, a presentation with the findings from an advanced illness program. These slides were presented at this year's NHPCO meeting by Advisory Board member Danielle Butin.
